Basic Function

Performs basic social work duties using social work principals to help in evaluating, planning and resolving the patient’s minor psychosocial problems or compliance with care in the in-patient units of DHC. Assures adherence to established JHAH policies, JCIA and CBAHI standards.

Scope

Responds to Consultation for Social Work Service made out by the General Practice, physicians and authorized medical staff for providing counseling, performing home/social evaluation, discharge planning, etc. for patients receiving medical treatment in the in-patient units of DHC involving the patient relatives. Also responsible to document the outcome in Epic. Makes adequate follow-ups for continuity of counseling. Adheres to Confidentiality of Patient Information. Participates in social work activities as directed by Manager of SWSU. Receives supervision and guidance from the professional Social Workers and Service Manager for appropriate handling of complicated cases. Reports to the Manager, Social Work Services.

Principal Contacts

Direct contacts with in-patient medically eligible cases and their families at in-patient units of DHC. Direct contacts with the referring physicians, and the relevant healthcare workers involved with the case, and/or persons of significant importance to the patient’s illness.

Principal Duties

  1. Responds to Consultations for Social Work intervention from relevant healthcare staff  or case findings Interviews cases to assess factors (psychological, social, compliance or other concerns) affecting the patient present plan of care.
  2. Makes psychosocial assessment and counseling of the referred patients and their relatives that have relationship with present problem.
  3. Discusses/consults with and/or informs concerned medical staff regarding psychosocial factors and other aspects to assure integrated plan of care.
  4. Documents all cases (initial and progress notes findings) in the patient EHR.
  5. Makes appropriate and interval follow-ups on the patients and their families as care required.
  6. Participates in the Daily/Weekly In-patient Unit rounds with other health care providers,
  7. Attends scheduled communication meetings, in-service training, etc.
  8. Prepares and submit reports covering assigned area of responsibility, such as, monthly reports, daily reports and assignment reports as requested.

Education

Bachelor degree in Social Work, Sociology or psychology preferred

Experience

Up to 3 years’ experience, preferably one year work experience in medical social work field.

Certifications/Other requirements as applicable

  • Acceptable commands of oral and written English language. Arabic Language is required.
  • Requires broad knowledge of the field of social work
  • Understanding of the cultural and social aspects in dealing with multi-nationals.
