Job Description Summary
The Associate Population Health Advisor is responsible for collecting and entering health data, working closely with senior staff to generate health feedback reports, assist in developing and implementing targeted population health initiatives aiming to improve their health outcomes.

- Collect validate healthrelated data from various sources including electronic health records EHRs health risk assessment surveys and Health registries
- Maintain and update databases to ensure data accuracy and integrity Pull reports and proper documentation that can be used for assessing PHM needs M13
- Support health initiatives and prevention programs in collaboration with multidisciplinary team across JHAH and Network providers
- Utilize JHAHNetwork Providers health needs assessment data to identify opportunities for population health management initiatives
Minimum Education Required
Bachelor’s degree Public Health, Health Promotion, Health informatics, Nursing or related health sciences
Professional Certifications Required

Years of Experience
Up to 2 years' experience in Population Health
Languages
