Job Description Summary

The Case Manager 3 is responsible for the facilitation, coordination and transition of patient’s plan of care from one healthcare environment to another, or home, based on their need for continued care – which includes but is not limited to coordinating the assessment, planning, implementation, intervention and evaluation of the options and services available to meet the patients’ healthcare needs across an episode or continuum of care; advocating for patient wellness, autonomy and resource management and monitoring clinical resource utilization – in order to connect patients with appropriate healthcare providers and human services and care settings, whilst ensuring that the care provided is safe, effective, patient centered, timely, efficient and equitable.

  • Actively participate in the development and implementation of Case Management projects including Clinical Pathways taskforce committees quality improvement tools data analysis and variance reports
  • Maintain personal professional development as a Case manager keeping abreast with clinical changes and evidencebased practice
  • Mentor and orientate new or less senior staff about case management roles and responsibilities

  • Complete an initial screening of all assigned patients to identify patient strengths and needs related to clinical resource utilization and discharge planning
  • Communicate effectively between the physician and other services to improve coordination of care
  • Assist patients in setting specific measurable achievable realistic and timebound SMART goals for selfmanagement
  • Set patients and caregivers expectations measuring and encouraging willingness to be involved with a plan of care and treatment goals
  • Actively works to eliminate barriers to the delivery of clinical services with the patient family and healthcare team and monitors clinical resource utilization
  • Support patients progress intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused and efficient
  • Conduct follow up contact with patients care givers as indicated to ensure compliance with recommendationsProvide telephone advice per protocol handling urgent calls and emergent calls and refertroubleshoot as necessary
  • Maintain a list of medical supply and resources available to patients and maintaining collegial relationship with the entities used most frequently
  • Coordinate the patients plan of care and monitors multidisciplinary patient care activities to develop appropriate care delivery strategies for identified patients and make recommendations as required Ensures that interventions are aligned with the medical management initiatives
  • Creates a discharge plan appropriate to the patients needs and resources Based on the individual needs and values of the patient and in collaboration with all service providers the care manager connects patients with appropriate healthcare providers services and care settings whilst ensuring that the care provided is safe effective patient centered timely efficient and equitable This approach achieves optimum value and desired outcomes for the patient families healthcare providers and the organization reduce length of hospital stay and reduce readmission rate

Minimum Education Required

Bachelor's Degree in Nursing required.

Professional Certifications Required

Current licensure as a professional registered nurse. 7 years minimum clinical nursing experience following licensure/registration in the country of origin, to include 2 years Case Manager/case coordinator experience preferred. Demonstrated strong Oral and Written Communication Skills, Project Management, Quality Improvement, Change management, Time Management and Organizational skills are required.
